Both purchase and top-up seem to be available. You can apparently buy one at the JR EAST Travel Service Center next to the Tokyo Monorail gates in Terminal 3, or at Welcome Suica-compatible ticket machines in the same terminal. The short-stay "Welcome Suica" (valid for 28 days) also appears to be sold there. Top-ups can be done right at the station ticket machines, from what I've seen.
